Care Management
A care manager who speaks your language, coordinating your family's services in one place.

Understanding Care Management
Care management is ongoing support from a care manager who helps coordinate your family's health care, services, and appointments. In New York, care management is provided by Care Management Agencies (CMAs) that work with Health Homes. It's available to children and adults with Medicaid who have qualifying chronic health or behavioral health conditions.

A care manager helps connect the pieces — doctors, therapies, home care, and benefits — so your family isn't doing it all alone.
