Home Care for Children & Adults
A trained home health aide can help your loved one with daily care at home. We help you find the right program for your family.

Understanding home care
Home care means a home health aide comes to your home to help with daily activities like bathing, dressing, eating, and moving around. In New York, there are three main ways to get home care, depending on age, insurance, and OPWDD approval.
Compare the three programs
| Program | Who it's for | Typical age | Hours |
|---|---|---|---|
| HMO | Children and adults with a Medicaid health plan | 4+ | Usually limited |
| Pediatric CHHA | Children with OPWDD approval | 4–18 | Based on your child's needs |
| MLTC | Adults with disabilities or chronic illness | 18+ | Based on assessment |

Three ways to get home care
Home Care Through Your Health Plan (HMO)
Who it's for
Children and adults enrolled in a Medicaid health plan (HMO), such as Healthfirst, MetroPlus, Anthem, and others. No OPWDD approval needed.
What you get
A home health aide for daily care. Hours are usually limited, so this is often a good starting point while other applications are in progress.
Pediatric Home Care Through a CHHA
Who it's for
Children with OPWDD approval.
What you get
A home health aide through a Certified Home Health Agency (CHHA). Hours are based on your child's needs and aide availability, and are often much higher than HMO home care.
Managed Long Term Care (MLTC)
Who it's for
Adults with disabilities, chronic illness, or complex medical needs who need ongoing help at home.
What you get
A home health aide plus a plan that coordinates long-term care. Eligibility and hours depend on your assessment.
